Hybrid Coordinator (Patient Care & Scheduling)

Position Summary

The Hybrid Coordinator combines the responsibilities of a Patient Care Coordinator and Scheduling Coordinator to support healthcare providers and patients in a fully remote setting. This role requires a licensed Registered Nurse (RN) who can coordinate patient care, manage scheduling activities, facilitate communication between patients and providers, and ensure efficient daily operations while maintaining exceptional patient service.

Key Responsibilities

Patient Care Coordination

  • Provide virtual assistance to patients via phone, email, and video calls.
  • Conduct initial patient assessments, including reviewing medical histories and current medications.
  • Offer compassionate guidance and support to patients and their families.
  • Schedule and coordinate patient appointments and follow-up visits.
  • Arrange laboratory tests, imaging, and specialist referrals.
  • Maintain accurate and confidential electronic patient records.
  • Educate patients on treatment plans, medications, and follow-up care.
  • Ensure HIPAA compliance and patient confidentiality.
  • Assist with billing documentation and insurance claims.
  • Participate in quality improvement initiatives and collect patient feedback.

Scheduling Coordination

  • Manage and update Intellicure EMR for accurate patient scheduling.
  • Handle inbound calls from providers and team members.
  • Maintain calendars, schedules, and provider availability.
  • Confirm appointments and obtain patient consent.
  • Send appointment reminders and reschedule canceled appointments.
  • Manage workflows and task tracking using Monday.com.
  • Monitor shared email inboxes and respond promptly to inquiries.
  • Maintain provider censuses and coverage maps.
  • Review assigned provider notes for accuracy.
  • Coordinate provider shadowing schedules.
  • Provide administrative support and basic technical troubleshooting as needed.

Qualifications

  • Current and valid Registered Nurse (RN) license.
  • Minimum of 3 years of nursing experience.
  • Experience in patient care coordination preferred.
  • Healthcare Virtual Assistant experience is a plus.
  • Strong clinical, communication, and organizational skills.
  • Experience using Electronic Health Record (EHR) systems.
  • Familiarity with scheduling platforms and workflow management tools such as Monday.com is preferred.
  • Ability to multitask and manage multiple priorities in a fast-paced environment.
  • Excellent problem-solving and customer service skills.
  • Compassionate, patient-focused, and detail-oriented.
